CHILDHOOD ARRIVALS DOCUMENT CHECKLIST

INTAKE FEE – $100 (cash or money order)

USCIS FEE – $465 Money Order made to USCIS (NO CHECKS!)

2 Color passport photos

List of Addresses since entering the U.S., as best as you can remember, AND THE Dates that you Lived there (the beginning and ending dates)

 

*  IDENTITY (one of the following)

  • Passport
  • Birth Certificate and Photo Identification (we can translate the birth certificate if you do not have a translation)

 

*  ENTRY PRIOR TO 16TH BIRTHDAY (one of the following)

  • School Records
  • Medical Records
  • Stamped Passport or I-94 Arrival / Departure Document
  • Notice to Appear from Immigration

 

*  PRESENCE IN THE UNITED STATES ON JUNE 15TH 2012

  • A document that shows you were here dated June 15th OR
  • Several documents dated June 2012, before and after the 15th.  This may be debit card records, bank transactions, or cell phone record. Or a Photocopy of a sign-in sheet for a gym or a pool or a worksite or your child’s daycare

 

*  DOCUMENTS FROM EACH MONTH BETWEEN JUNE 2007 TO TODAY THAT SHOW YOU LIVED IN THE UNITED STATES.  ONE DOCUMENT MAY COVER SEVERAL MONTHS (like a lease agreement) OR SEVERAL YEARS (LIKE A SCHOOL TRANSCRIPT) (Bring as many as possible)

  • School Transcript
  •  Employment Records (or detailed letter from employer)
  • Medical Bills, Household Bills, and Postmarked Letters
  • Marriage Certificates and Birth Certificates of Children
  • Rental Agreements
  • Banking Records and Tax Receipts

 

*  SCHOOL REQUIREMENT (bring as many as possible)

  • If you are currently in school, bring a copy of your school schedule, recent progress report, or letter that states you are attending school
  • High School Diploma or GED
  • OR Military Honorable Discharge Records

 

*  ACCOUNTING FOR ALL DEPARTURES FROM THE UNITED STATES SINCE JUNE 15, 2007

 

*  Transportation Ticket,, Passport Entries or Advance Parole document, Records from why you left the country, such as wedding, funeral, or medical emergency

 

*  CRIMINAL RECORDS.  If EVER arrested, you must bring ALL arrest and court documents.
